I just got around to posting this. I hope some auto techs that maybe work at dealers might be able to help. This strange specialized nut that is a little bigger than a quarter in diameter. I put my car up on my lift and still don't see anything similar. This fell out when I pulled the front driver side inner fender liner.

I don't know, but it might be one of the old coke bottle inside the door panel? Fasteners do sometimes get knocked into place they weren't meant to be.

Closest thing i could find with google vision was an axel nut

@ Farmer Fran I first considered an axel nut also but the material would not hold up to the high torque value that would be needed.

On another car that I have, I pulled a fender liner that had a hole in it to replace it... a big bolt fell out... likely what put the hole in there in the first place. This was like 8 years ago, no problems at all.
