When the tach stalled what did the engine do? Did it start missing pulling timing fall flat? Odd part is the first gear relationship.

Sometimes one will need to use a tad bit more aggression getting back into/up to speed after pulling over onto the shoulder of the freeway/highway [in order] to verify the noise you heard was not a loose lug nut.
